Following undergraduate (Physics, Cornell University), medical (SUNY, Stony Brook), general surgery (Cornell/New York-Presbyterian Hospital), and complex surgical oncology (Memorial Sloan Kettering, MSK) training, Balachandran joined MSK as an independent surgeon-scientist in the Human Oncology and Pathogenesis Program (HOPP, 2017). He is currently Associate Member and Director of The Olayan Center for Cancer Vaccines. In just 8 years, through 6 consecutive first/senior author Nature papers, Balachandran pioneered the discovery and clinical development of new immunotherapies for pancreatic cancer, a deadly cancer long presumed to be immunotherapy-insensitive.

Balachandran has challenged the prevailing belief that pancreatic cancers harbor few clinically relevant antigens. In 2017, he demonstrated that rare pancreatic cancer patients naturally mount T cell immunity against mutated neoantigens that may confer long-term survival (Nature, 2017). He then identified the immunogenic features of these neoantigens (Nature, 2022) and revealed previously unrecognized immune cells (group 2 innate lymphoid cells; Nature, 2020) and mechanisms (IL33-induced lymphoneogenesis; Nature, 2025) that enhance anti-tumor T cells in pancreatic cancer. These discoveries upended the assumption that pancreatic cancer is “immunologically inert” and illuminated novel paradigms to treat pancreatic cancer with immunotherapy

Balachandran then swiftly moved these discoveries to clinical breakthroughs, developing and testing the first bespoke RNA neoantigen vaccine for pancreatic cancer. Through a landmark clinical trial, his team demonstrated vaccines induce long-lasting T cells that may delay pancreatic cancer recurrence (Nature, 2023; Nature, 2025). As neoantigens are oncogenic byproducts of all cancers, these findings have ignited global interest in RNA vaccines as potentially transformative treatments for pancreatic cancer and other deadly cancers. Balachandran then founded and directs The Olayan Center for Cancer Vaccines at MSK, a leading-edge innovation hub focused on establishing the principles and practice of next-generation precision immunotherapy.

Balachandran has received several honors, including the 2023 Trailblazer Prize for Clinician-Scientists from the Foundation for the NIH.
